HIM Student Resources

Student Enrollment Packet
The following items are required by all HIM students and must be received before the first day of class:

• Background Check
  
  
• MO/DL SS# release form/signature
  
  
• Tablet PC Information
  The fee for the tablet PC will be applied to your account within the Enroll & Pay system. Half of the total will be applied to your first semester in the program, and the balance due will be applied to your second semester of class.
  
  By late July, the tablet PC fee should be included in your account and students may apply for financial aid. Complete the Budget Adjustment Request form by noting the full cost and submit to Angie Dopp in KUMC Financial Aid office. No receipt is required.
  
  Tablet PCs will be distributed on the first day of student orientation.
